Assertive Community Treatment

Assertive Community Treatment - ACT

ACT helps people with severe and ongoing mental health challenges who may have difficulty living on their own. Most ACT services take place in the community or in the person's home. Individuals are also encouraged to include family or friends in their treatment when possible.

Care Coordination

Care Coordination

BCCMHA Care Coordinators help by connecting individuals to other providers and services (both internally and through local community partnerships), scheduling and following up on appointments, and assisting with program eligibility and enrollment. BCCMHA Care Coordinators support individuals in managing their care, finding resources and benefits, and navigating transitions they may need between services. Care Coordinators are also here to be a person's advocate and help them reach their goals; both in health, wellness, and life.

Care Management

Care Management

Care Managers support an individual's health journey. They personally work with each person to set and achieve health goals, connect care with other healthcare providers, manage an individual's medications, and complete important health screenings. Care Managers also find the best ways to manage chronic conditions like diabetes, high blood pressure, obesity, and pain.

At the start of services, every new client receives a primary care screening, along with education on medications and health conditions when needed. This helps make sure that all care providers are working together to give each person the best possible care.

Case Management

Case Management

Case Management helps adults connect to community resources, keep track of their needs, and coordinate services. Case Managers also provide support and advocacy when needed. Case Managers also assist adults in getting the right referrals within both BCCMHA and throughout Barry County.

Community Health Workers

Community Health Workers (CHWs)

Community Health Workers, CHWs, serve as a vital link to improve access to healthcare, promote healthy behaviors, and support individuals through connections to resources. CHWs work with individuals on a short and longer term basis to assist with health education, outreach, systems navigation, advocacy, and referrals to community resources. 

Community Based Services

Skill Building

Skill Building services develop and enhance specific skills that enable individuals to function more independently in various aspects of daily living. They teach skills related to personal care, household tasks, communication, social interaction, vocational skills, and other areas necessary for self-sufficiency. The goal of skill building services is to promote skill acquisition, practice, and mastery.

Community Living Supports (CLS)

CLS helps adults with mental health needs live more independently in their homes and communities. CLS services provide assistance with daily activities such as cooking, cleaning, personal care, and managing money. CLS also supports social skills, helps adults get involved in community activities, and works on building routines that promote independence. The goal of CLS is to improve quality of life by giving people the skills and support they need to live more comfortably and confidently in their everyday lives.

Consultation and Prevention Services

At BCCMHA, we believe in the power of support and teamwork! We partner with other local organizations to provide education and raise community awareness, helping to prevent everyday challenges and stress from turning into serious mental health concerns. Our prevention programs are designed to make a positive impact, and some of these services are supported by the Barry County United Way.

Employment Services

Supported Employment

Supported Employment services help individuals with a wide range of employment related activities from resume building to job seeking, to interviewing skills, job placement, job retention, and job coaching. The goal is always competitive integrated employment, fair wages, and finding a job that fits each person’s individual needs. 

Benefits Coaching

A Benefits Coach is available to help individuals with concerns about the impact work may have on benefits such as Social Security Disability benefits, Social Security income payments, and Medicaid/Medicare. Ongoing assistance can be given for help reporting for a job or addressing concerns as they arise.

Customized Employment

Customized Employment is designed to meet the specific abilities of an individual with a significant disability or higher support needs and the business needs of an employer and find flexible ways to develop a job that suits everyone’s needs, strengths, and interests while also ensuring that the individual has competitive integrated employment.  It is a very individualized approach to obtaining meaningful work while considering the very specific needs of the person and business.

Older Adult and OBRA Services

Older Adult and OBRA Services

At BCCMHA, we provide older adults and senior citizens with mental illnesses or developmental disabilities a behavioral health assessment while they are in a skilled nursing facility. If services are deemed necessary, care can be provided within the skilled nursing care facility or by BCCMHA.

*OBRA is a federally mandated program that requires persons with mental illness or developmental disability to be assessed prior to nursing home placement to ensure that needed mental health services are provided during placement. OBRA includes pre-admission screening and annual resident review.

Outpatient Therapy

Outpatient Therapy

Outpatient therapy services are available for adults of all ages who need support with behavioral health, substance use disorder and/or co-occurring (dual diagnosis) disorders. Specialized outpatient therapy is also available for veterans and trauma support.

Outpatient therapy may include group and individual sessions, tailored to fit a person's needs. For those experiencing more urgent symptoms, Intensive Outpatient Services are available. We are dedicated to offering quick and effective solutions to help you stabilize, recover, and move forward in life!

Peer Support Specialists

Peer Support

Adult Peer Support services are provided by people (commonly called "Specialists") who have gone through similar life experiences as the adult they are helping. People with mental health or substance use disorders have a special ability to support each other because they share similar experiences. By sharing their own stories, they offer support, strength, and hope to others, helping them grow, improve their well-being, and recover together.

Why Have Peer Support Specialists?

A BCCMHA Peer Support Specialist is a person with a significant life-altering experience. This is also referred to as a "lived experience." These specialists support adults with struggles pertaining to mental health, psychological trauma or substance use disorder. Because of their lived experience, these specialists have an expertise that professional training cannot replicate.

Psychiatric Care

Psychiatric Care

Psychiatric care provides psychiatric evaluations and monitoring of psychiatric services and medication for adults with serious and ongoing mental health issues. Care is given by psychiatric doctors and nurse practitioners. Services are offered in-conjuction with additional BCCMHA services and care.

Respite Care

Respite Care

Respite care supports individuals in staying in their own homes. This short-term service gives family members or primary caregivers a break from their daily caregiving duties, helping to reduce stress and maintain a healthy home environment.

Veteran Services

Veteran Services

We are committed to providing behavioral health and wellness services and assistance to those distinguished men and women, who have honorably served and protected our country.

Veteran Navigator Services is available to any veteran and provides support to both the veteran and their military family members facing emotional challenges that require guidance and resource linkage. Veteran Navigator Services assists in maintaining secure housing, addressing employment concerns, transportation, medical needs, management of personal finances, personal care requirements, food resources, obtaining military documentation, and more.

BCCMHA has an assigned Barry County Veteran Peer Navigator to serve those who served our country, free of charge.
