Applied Behavior Therapy focuses on improving specific behaviors, such as social skills, communication, reading, and academics as well as adaptive learning skills, such as fine motor dexterity, hygiene, grooming, domestic capabilities, punctuality, and job competence.

Care Coordination

Care Coordination

Care coordinators help by connecting youth to other providers and services (both internally and through local community partnerships), scheduling and following up on appointments, and assisting with program eligibility and enrollment. BCCMHA Care Coordinators also support youth in managing their care, finding resources and benefits, and navigating transitions they may need between services. Care Coordinators are here to be a child's advocate and help them reach their goals, both in health, wellness, and as they grow through the years.

Care Management

Care Management

Care Managers support youth and their families on their health journey. They personally work with each member to set and achieve health goals, connect care with other healthcare providers, manage the youth's medications, and complete important health screenings. Care Managers also find the best ways to manage chronic conditions like diabetes, high blood pressure, obesity, and pain.

At the start of services, every youth and young adult receives a primary care screening, along with education on medications and health conditions when needed. This helps make sure that all care providers are working together to give children the best possible care.

Case Management

Case Management

Case Management helps youth and young adults connect to community resources, keep track of their needs, and coordinate services. Case Managers also provide support and advocacy when needed. Case Manager's assist individuals in getting the right referrals within both BCCMHA and throughout Barry County.
